The Senior Alliance Information and Assistance Team

The Senior Alliance, the Area Agency on Aging for Region 1-C in the Wayne County area, offers care management and Information and Assistance services that help families coordinate the support a loved one needs to remain safely at home. Care management gives families a professional to assess needs, build a plan, and arrange and monitor services on their behalf.

Through care management, a professional evaluates the person’s situation and coordinates in-home and community services — meal delivery, in-home help, personal care, transportation, and the MI Choice Medicaid Waiver, among others — adjusting the plan as needs change. An Information & Assistance Specialist is the free entry point, and the agency’s Caregiving Haven platform offers additional tools, including a customizable caregiver binder to help families organize care information. Professional coordination replaces a fragmented, self-managed effort with a sustainable plan.
