Diabetes PATH
Are you living with pre-diabetes or managing Type 1 or Type 2 diabetes? Diabetes PATH (Personal Action Toward Health) is a dynamic 6-week, 2.5-hour-per-week interactive workshop offered online via Zoom that equips you with practical tools to live healthier and do more of what matters.
From managing blood sugar levels and crafting balanced meal plans to reducing stress, navigating difficult emotions, and taking steps to prevent long-term complications—this program supports you in setting your own goals, fostering better habits, and improving your overall well-being.
Caregivers and loved ones are also encouraged to join in as active participants.
